This is not a sunlight photo but as a birth photographer I don't get to place my subject in a specific position and I am not allowed to use a flash in the operating theatre.  This week I photographed a baby's birth at 33 weeks gestation. The sharp overhead lights created the shaping you mentioned in the class.
