This is an example of how I typically take and enhance my notes.  I write the areas of focus under the Subject.  I liked how Kristopher had the dividing lines so I added one below my Focus.  For the first half, I give the example of when a Focus requires specific steps.  I use a curved arrow indicating each successive step.  In an enclosed box, I use an arrow with two lines on the "stem"  to indicate something that requires further research.

The second Focus just has a few things important to remember, so they are just numbered.  

Finally, I have another enclosed rectangle, this time with a Question Mark in the beginning, and typically I would write the question I have after it.
