For this class project, you will design a complete Google Form survey based on your own thesis or academic research topic. This hands-on task will help you put theory into practice and prepare a real, usable survey for your project or proposal.
Your Project Should Include:

A clear and meaningful survey title

A short description with ethical consent information

At least 2 demographic questions (e.g., age, gender, major)

At least 5 topic-related questions, including a mix of:

Multiple choice

Text

Checkboxes

At least one use of logic branching (e.g., skip questions based on previous answers)

Use of sections to organize the form (Consent, Demographics, Main Questions, etc.)
What to Submit:

Link to your Google Form (make sure it’s set to view-only or allow responses)

Or upload screenshots of your form layout

Include a link to your connected Google Sheet with at least a few rows of dummy response data
