1. Introduction: If you're running
a small business, you have probably
dealt with the mess of creating and
collecting forms, client testimonials, client
feedback, intake forms, product suggestions,
whatever it is, all types of forms kind of being scattered
all over the place. In the past, I have used a combination of
Google Forms and just honestly emails or I get testimonials
through DMs and such. But the thing is,
everything else administratively for my
business lives on Notion. So it just makes sense for
me to move those forms and collecting of all forms
to go to Notion as well. Now, with Notion
Forms, I can do that. So I have now moved
everything over, and it is all living
in one happy place. By the way, hello, hello. I am Cheryl. I am a brand
and website designer, and you may have
some of my videos over on my YouTube channel
called Made on Sundays. And in this mini class today, we are going to be
looking at how to create forms on Notion,
and in the end, you will have a fully
working, beautiful, sharable notion form
that you can embed or send and share to whoever
you like on the Internet. So let's get into it.
2. Create a Notion Form: Today's example,
we're going to be creating a client
testimonial form. So how Notion Forms work on Notion is it is
based on a database. So in order to create this form, you do need to create
a database and host it somewhere
on your notion. For a client testimonial, it's usually client facing, but you don't necessarily
want to create the database so that your client can view
all of the answers. So you want to make sure
that you're creating the database on your
own internal side, and then once you're
ready to share the form, you can share it into
directly to them as a link, or we're going to
talk about all of the sharing options
in the next lesson. But for right now, we are going to be using
our home dashboard. This home dashboard, by the way, is a free template that you can download on the
class workbook. So if you want that, you can go ahead and find that
template there. It's part of my bigger
client HQ system. Right here, if you scroll down, you'll see a little
workspace that I have created under here is a page
called client testimonial. Anything testimonial
related will be saved here. We're going to head
into here and we're going to create our first form. To create a form,
you can either click Add and then find
Form or you can forwards Form and
then click on Form. Just like that,
we're going to have a new form created.
You can name it. And then you can give
it a form title, testimonial, then you can go ahead and add
your questions in. Anywhere you see this blue
plus sign is where you can add your questions in and you
can also obviously move them around and adjust
it afterwards as well. I'm just going to add a
question here right now, and there are many options
of different types of questions or answers you want to receive
from your form. The most notable and common two are probably going
to be the first top two, which is text and
multiple choice. Let's go through those together. First we have text. You can type in your
question. What is your name? Then they would essentially
answer in that box. If you click on it or
on these three dots, you can change some of the
settings here as well. You can click on required, which requires them to answer
before they can submit. You can add a description, or you can even make
this long answer. For example, if you wanted
to add a question like, tell me about your business. Then that might be
a longer answer, which requires a bigger box and you can add that
in just like that. Then we have our multiple
choice questions. Again, we're going to
click on Add a Question and we're going to click
on multiple choice. This is the type of
question when you want to ask yes or no questions or choose one of the below or rate this service like one star out of five star,
that kind of thing. So for example, if you want
to do a yes or no question, you can say, Are you
happy with the service? And then it'll be yes or no. Or if you want to do more than two questions
or more than two answers, then you can type in something
like rate My service. And then you can add
text or even Emoji. Maybe a star system. Let's say we have one star here, and then we can add two star, three star, four
star, and five star. Again, if you click on the
three dots right here, all right here, you can actually
choose a max selection. This is really important. It allows people to choose a maximum of certain
amount of selection. Let's say for this question,
a yes or no answer. You don't want them to be able
to click on both answers. Then in this case, we would allow them to answer
one question. But if you have a
question where, let's say you allow
multiple answers, then you can go ahead and
adjust that Mac selection to Unlimited or whatever the
amount of selection is allowed. Other things that I think might be interesting to
add into a form, I always like to include
some probably your email. That one's a really, um, straightforward one, but
also files and media. A lot of times when I ask
for client testimonials, I will ask whether
they are okay with me sharing their
testimonial on my website, and if they click on yes, then I would love them to provide some headshot
or something so that having this allows them to upload a headshot
of some sort, and then I can use
that headshot, download it and added to my
website testimonial page. When you are done
with adding all of your questions in at
the top bar here, you're going to see a
couple of things that we can look at and adjust. If you click on responses, this is where all of the
responses will appear. If you send this form
to multiple clients, all of their answers
will then be populated into the
space right here. You click on this little
button right here, you're going to be able to adjust your
submidscreen as well. The submit screen is when they click on the submit
button, what will they see? First off, you can change
the color of your button. Let's say you want to
change it to yellow, you can do that or
whichever color you like. You can change your
submit button text. Instead of saying just submit, you can be like, let's go
or something like that. And then you can have a little
once I click on Summit, you'll have a confirmation
title and body description. You can also fill those in. You can say something
like, thank you, and then you can maybe
say something like, I will get back to you in three days if this was a client
intake form or something. So a few other things
that you can adjust here. They might be able to
view their responses. They might be able to, um, email a copy to themselves, then this will also allow
them to submit another one, they're allowed to submit
multiple things, for example, if you're asking for product ideas or
something like that, you might want to allow them
to submit multiple answers. Once that is done, if you just
put your cursor over here, you'll be able to change
the form icon as well. Let's say you want to put a little a writing Imoge
or I don't know, for client testimonials, I typically like to
use this Emoji. You can add a form cover, and you can change
that image right here. You can either upload
something or maybe you can look up something
on splash. There we go. And you can also add a little description
for the form as well. So once you are done
with your form, you can click on this
button to preview it, and this is what it would
look to someone who is filling out your form and
then you click on Let's go. We'll need to type something
in here. Let's go. And then it will show
your submit response. Now in the next
lesson, let's look at how to share this
form with your client.
3. Share a Notion Form: Now for sharing this form, when you're ready to share, you can go ahead and click
on Share Form up here and you can fill out who
can fill out this form. Usually, you probably pick
anyone with the web link can fill out this form so that whoever you send
it to can fill it out. You can allow for anonymous
responses if you want. There are two ways to actually three ways that we're going to be
talking about today. First, there is the form link. This is a link that you
can click on copy here and send it over so you
can DM your client, email your clients or whatever you're talking
to your client, you can just send
them this link. So essentially, it will look
like this when they open it. It's just a browser with the form there and they
can submit their form there. Another way of sharing this form is you
still copy that form. But let's say you want to
embed it within their client. Portal or inside some
other Notion space that you have, you can go ahead. This is my client portal that I usually
send to my client. Every single one of
my client projects, there is a space like this in order for us
to work together. Inside, I usually have
a little space called useful resources and
under testimonial. This is probably where I
would want to embed the form. Instead of creating
a manual form here, I'm going to actually just just paste it into
this page and we can click on embed and I will actually embed that form right into
the space right here. Um, you can either embed
it into this space or another option would
be to make a button. So let's say you want to
create a button instead, you can go to forward
slash Button. And we're going to
create a new button, and we're going to
call this button, submit your testimonial. And then what it's going to do is when this button is clicked, this is the action
that will be taken, which is to open a new
page and form or URL. We're going to paste in
this link right here, and I would recommend clicking on your internal link right here and then click on Done and once you
click on this button, it will actually open up it opened up this
page right here. Really cool way to share
your form as well, and you can change the
color of this as well to another color so that you
can see it a bit easier. All right. Those
are the two ways. The last way to share your
form that I'd like to share is actually through
embedding this form into, let's say, your show at website. Um, so all you need
to do is go to embed this form and copy
this code right here. And I'm going to
bring in my SHOWED um, website right here. Let's say you want to embed
this form on this page on your SHOW website
or whatever website your um website
platform you're using. You're going to do
is you are going to go to add element
and embed code. Then right here, I'm going to paste that frame
that we just copied, and then you can adjust
the size of your form. Let's say it's
something like this. When you're ready
and it looks good, you can click on preview, and let's see how that looks. Perfect. That is
the last way on how to share your form quickly and easily through Notion Forms.
4. Class project: Now that you have your form all built out and ready
to be shared, here's what I'd like you to do. For your class project, build one form on Notion Forms. It could be a client
testimonial form like we did a feedback form or a
product suggestion form. Whatever feels useful for
your business right now, just create it, add
some questions in, and then embed it somewhere, embed it as a
Notion button or on a different Notion page or on your website,
wherever you'd like. Then take a screenshot of it and share it in the
project section. I would love to see
what you create and how you customize it and personalize it for
your business, and I'm sure other
students would also appreciate seeing
different variations. If you want a head
